Finishing Forms

 

If you wish to finish your piece with us, please print the appropriate form below-please contact us if you have any questions which one to use.  

When shipping to us, please send with tracking.  UPS is generally the most reliable, with a close second via USPS.  Fedex and DHL are hit or miss.  Please make sure to wrap in a water proof bag before inserting into box or envelope.  Please reach out to us if you need a shipping label.

When completing the forms, please be as detailed as possible.  Once it arrives in the shop, we will call to confirm and clarify any other questions we have.  We will also clarify a price estimate.
